New Form Required on Indent pending PO

Dear Sir/Mam,

Kindly create a new form in IMIS for "Indent Pending for PO Status" in the manner shared in the attached excel file.

Also please Deactivate the Report - IMSR3265 as we have reviewed it and the report is not appropriate for our use.
