ERROR DURING REQUISITION OF GHAN ITEMS-075 CODE

Dear Sir/Madam

We are able to put requisition in Ghan Requisition item for raw material.
Two errors are occurring.
1. Unique Constraint( IMISMAIN.UK_REQS ) violated.
2.DS1120L-006:-Error code not define.

Screen shot of both have been attached , Kindly check it and resolve it asap.

CODE:-075 Snacks division.
